NIH Announces new “FORMS-E” Grant Application Forms and Instructions, Effective January 25, 2018

NIH has announced that there will be changes to its grant application forms and application guide instructions for all proposals with due dates on or after January 25, 2018. The new “FORMS-E” package will include the consolidation of human subjects, inclusion enrollment and clinical trial information, the expansion and use of discrete form fields to collect additional clinical trial information, and the incorporation of recent Grants.gov changes to the R&R Budget and SBIR/STTR Information forms.

A summary of the NIH changes on the FORMS-E package is located at:  https://grants.nih.gov/grants/funding/high-level_summary_of_form_changes-FORMS-E.pdf .

NIH will publish new funding opportunity announcements (FOAs) with FORMS-E application packages on October 25, 2017.  If the FOA was published before this date, it will be posted without application forms.  The application forms will be added to these FOAs by November 10, 2017.

On or after January 25, 2018, all Cayuse proposals that are created should use the new FORMS-E application package for all application types (New, Resubmission, Renewal and Revision). Even if the application is forwarded to OSP early, if the NIH deadline is on or after January 25, 2018, the FORMS-E package must be used.

If you have already created an application in Cayuse on the outdated application package, you can convert to the new FORMS-E package by using the TRANSFORM feature.  When using the TRANSFORM feature, the proposal will be created applying the most current application package.  If you have questions about using the TRANSFORM feature in Cayuse, please contact your OSP Analyst.

In anticipation of the transition to FORMS-E, please familiarize yourself with the NIH table identifying the correct form package to use:
