Emergency Alert: A Guide to Reporting Work-Related Injuries

The Environmental Health and Safety Office (EHSO) is committed to ensuring the safety and health of Emory employees. In compliance with the Occupational Safety and Health Administration (OSHA) regulations, it is crucial to promptly report any serious occupational injuries or life-threatening emergencies. Outlined below are the specific reporting requirements for severe injury events, including work-related fatalities, hospitalizations, and amputations. Adhering to these guidelines ensures timely medical treatment for the injured employee and allows EHSO to meet the regulatory reporting requirements.

OSHA Reporting Requirements for Serious Occupational Injuries

Employers are required to report severe injury events, as described below, to OSHA within the timeframe shown. At Emory, all notifications to OSHA are managed through the EHSO.

  • All work-related fatalities within 8 hours.
  • All work-related in-patient hospitalizations of one or more employees within 24 hours.
  • All work-related amputations within 24 hours.
  • All work-related losses of an eye within 24 hours.

Work-related Hospitalizations, Deaths, and Life-Threatening Injuries or Illnesses

  • If an Emory employee is admitted to the hospital or experiences a work-related event that leads to an amputation, eye loss or death, the supervisor or designated responsible person must immediately notify Occupational Health Services (OHS): Page the on-call nurse practitioner @ 404-686-5500, enter ID# 50464.
  • If there is a life threatening emergency, such as a loss of consciousness, severe burn, open wounds, shock, amputation, or head injury, dial 911 and have the employee transported to the nearest Emory Hospital. The employee, or their supervisor if the employee is receiving medical treatment, should also notify OHS and complete an injury report as soon as possible.

Work-Related Injuries or Illnesses (Non-Emergency)

For reporting non-emergency incidents, such as work-related cuts, scrapes, minor burns, contusions, or needle sticks, the employee should notify their supervisor and complete an incident report in the Health and Occupational Management at Emory (HOME) portal, accessed via the Human Resources Self-Service portal. OHS will follow-up with the employee to gather more information or schedule an appointment. The employee or supervisor can call OHS directly if they have immediate questions following an injury or if the employee is unable to complete the incident report in HOME.

  • During business hours (Mon – Fri, 8:00am – 4:00pm): Contact the OHS main office @ 404-686-8587
  • After 4:00pm, weekends and holidays – Notify the OHS on-call nurse practitioner @ 404-686-5500, enter ID# 50464.

As a component of the injury management process, a representative from EHSO will also follow-up on work-related incidents by either requesting that the supervisor submit a Supervisor’s After-Action Report (SAAR) in HOME or by conducting an EHSO-led investigation.
