StARS Learning Collaborative
The StARS Center’s main goal is disseminating knowledge and evidence-based public health best practices for delivery of effective, high impact dementia care services. Additionally, the StARS Center aims to share knowledge and resources for identifying, linking, and sharing relevant data sources to promote program evaluation, health surveillance, and public health overall within and across states. The Learning Collaborative is an opportunity to engage, learn, and create partnerships within and across states.

Learning Collaborative attendees will gain an understanding about how the Maryland StARS Project standardized and connected data across statewide dementia, aging, health, and social service systems to strengthen bidirectional communication, referrals, and care coordination between community-based organizations and healthcare providers. The session will also highlight how integrated data can help quantify health outcomes and cost savings while enhancing the Maryland Department of Health’s dementia dashboard with information on dementia prevalence and the service needs of people living with dementia and their caregivers.

Virginia StARS Presentation and Discussion

The Virginia StARS Project webinar will discuss the goals and outcomes of Virginia Integrated Systems for Tracking Alzheimer’s Care (VISTA), a process designed to link and harmonize data collected by various dementia care management programs operating in the state. The presenters will share lessons learned in linking multiple state-supported dementia care management programs with the Virginia Memory Project (Virginia’s Alzheimer’s and dementia registry) to improve data sharing and enable analyses to guide state decision-making.
