Hard signs of vascular compromise dictate immediate surgical consultation. In the absence of hard signs I think a lot of us would start with ABI. This is a Ankle-Brachial Index and is a comparative ratio used to predict vascular injury. A huge part of the disposition of this patient should you chose to discharge would be discussions regarding compartment syndrome: what to look for, to immediately return to the ED, etc.
